Analysis of multidisciplinary team consultation of a patient with aortic dissection and sudden bilateral lower limb weakness and abnormal ST segment on electrocardiogram

Abstract:Objective To analyze the effect of multidisciplinary team(MDT)consultation of a patient with aortic dissection(AD)characterized mainly by sudden bilateral lower limb weakness and abnormal ST segment on electrocardiogram.Methods A retrospective analysis was conducted on the MDT consultation process of one AD patient with sudden bilateral lower limb weakness and abnormal ST segment on electrocardiogram as the main manifestations.Results The patient was a 39-year-old male with no obvious pain symptoms,but only presented with fatigue and mild chest tightness.The electrocardiogram showed abnormally depressed ST segment,hypokalemia,and significantly increased D-dimer.Cranial CT examination revealed multiple lacunar cerebral infarction foci and ischemic foci,which were easily confused with cerebrovascular diseases,hypokalemia,pulmonary embolism,lower extremity embolism,acute non-ST-segment elevation myocardial infarction and other diseases.After MDT consultation,the final diagnosis of AD was delayed by 1 h.He was then transferred to the Department of Cardiovascular Surgery of another hospital and died 24 h afterwards.Conclusion The clinical symptoms of AD are diverse.For patients with atypical symptoms and combined abnormalities in other auxiliary examinations,the severity of the disease often cannot be clearly assessed,which should attract our sufficient attention.Combining MDT consultation can effectively avoid delaying the patient's condition.
