Risk factors of persistent HPV infection in patients with grade Ⅰ cervical intraepithelial neoplasia after ALA-PDT treatment

Abstract:Objective To analyze the risk factors of persistent human papillomavirus(HPV)infection in patients with grade Ⅰ cervical intraepithelial neoplasia(CINⅠ)after 5-aminolevulinic acid photodynamic therapy(ALA-PDT)based on logistic regression analysis.Methods A retrospective analysis method was used to select 240 patients with CINⅠ who visited the hospital from June 2021 to June 2023 and all received ALA-PDT treatment.The patients were followed up for one year and grouped according to the reexamination results of HPV.Those with persistent HPV infection were included in the infection group,while those with negative infection were included in the normal group.The clinical data of the two groups were compared,and logistic regression was used to analyze the risk factors of persistent HPV infection in CINⅠ patients after ALA-PDT treatment.Results The patients were followed up for one year.Reexamination of HPV revealed that 57 patients(23.75%)of the patients had persistent infection and were included in the infection group.The remaining 183 patients(76.25%)changed from positive to negative and were included in the normal group.There were differences between the infection group and the normal group in terms of family history,smoking,number of pregnancies,HPV load before treatment,HPV subtypes,reproductive tract inflammation,multiple infections,and menopause(P<0.05,P<0.01).Family history,reproductive tract inflammation and menopause were found to have collinearity issues after examination,which,therefore,were removed from the model and analyzed again.Logistic regression analysis showed that the number of pregnancies,HPV load before treatment,HPV subtypes,smoking,and multiple infections were all influencing factors for persistent HPV infection in CINⅠ patients after ALA-PDT treatment(P<0.01).Conclusion ALA-PDT has a certain effect in the treatment of CINⅠ,but some patients still have persistent HPV infection after treatment,which may be related to the number of pregnancies,HPV load before treatment,HPV subtypes,smoking,and multiple infections.Clinical measures can be taken as early as possible to increase the negative conversion rate of HPV and improve the prognosis of patients.
