Relationship between TyG index and spontaneous preterm birth in patients with gestational diabetes

Abstract:Objective To explore the relationship between triglyceride-glucose(TyG)index and the occurrence of spontaneous preterm birth(sPTB)in patients with gestational diabetes mellitus(GDM).Methods Data of 273 patients with GDM admitted from January 2021 to January 2023 were retrospectively collected.According to the occurrence of sPTB,they were divided into a preterm birth group(n=92)and a full-term delivery group(n=181).The general data and laboratory index levels at the time of GDM diagnosis in the two groups were compared,and the TyG index was calculated.The relationship between the TyG index and the occurrence of sPTB in patients with GDM was analyzed.Results The pre-pregnancy body mass index(BMI),triglyceride(TG),fasting blood glucose(FBG),and TyG index at the time of diagnosis in the preterm birth group were all higher than those in the full-term delivery group(P<0.05).Multivariate logistic regression analysis showed that sPTB in patients with GDM might be related to the increase of pre-pregnancy BMI and TG,FBG,and TyG index at the time of diagnosis(P<0.05).The analysis of the restricted cubic spline model showed that the association between the TyG index and the occurrence of sPTB in patients presented a nonlinear dose-response relationship(P<0.05).When the TyG index was>9.3,the risk of sPTB increased with the increase of its level.Receiver operating characteristic(ROC)curve analysis showed that the area under the curve of the TyG index for predicting sPTB in patients was the highest,which was 0.808(95%CI:0.754,0.861).When the decision curve drawn by the combination of each index was within the threshold range of 0.17 to 0.87,the maximum net benefit rate was 0.337,suggesting that the model had good clinical practicability.Conclusion The occurrence of sPTB in patients with GDM may be related to the increase of TyG index.Therefore,early determination of TyG index can effectively predict the risk of sPTB.
