Analysis of Visual Function Recovery and Its Influencing Factors After Phacoemulsification Combined Lens Implantation in Patients with Diabetic Cataract

Abstract:Objective To explore visual function recovery and its influencing factors after phacoemulsification combined with intraocular lens implantation in patients with diabetic cataract.Methods A total of 106 patients with diabetic cataract admitted from June 2021 to October 2024 were retrospectively selected as the observation group,and another 110 patients with simple senile cataract during the same period were selected as the control group.All patients underwent phacoemulsification combined with intraocular lens implantation for cataract.The recovery of best corrected visual acuity(BCVA)after surgery was compared between the two groups.The patients in the observation group were further divided into the good recovery subgroup(BCVA>0.3 LogMAR,n=70)and the poor recovery subgroup(BCVA≤0.3 LogMAR,n=36)based on the BCVA at 3 months after surgery.The related factors affecting the recovery of visual function after diabetic cataract surgery were analyzed through univariate and multivariate logistic regression analysis.Results The improvement amplitudes of BCVA in the observation group at 1 week,1 month,and 3 months after surgery were all smaller than those in the control group(P<0.05).The proportions of age,duration of diabetes,nuclear hardness grade Ⅳ,preoperative glycated hemoglobin(HbA1c),preoperative BCVA,combined diabetic retinopathy,combined macular edema,and postoperative inflammatory response in the poor recovery group were all higher than those in the good recovery group(P<0.05,P<0.01).Multivariate logistic regression analysis showed that age,duration of diabetes,nuclear hardness grade,preoperative HbA1c,preoperative BCVA,combined diabetic retinopathy,macular edema,and postoperative inflammatory response were independent influencing factors affecting the preoperative recovery of visual function in patients with diabetic cataract(P<0.05,P<0.01).Conclusion The postoperative visual function recovery of patients with diabetic cataract is affected by multiple factors.Among them,age,duration of diabetes,nuclear hardness grade,preoperative HbA1c level,preoperative BCVA,combined diabetic retinopathy or macular edema,and postoperative inflammatory response are independent influencing factors.
