Relationship between Abnormal Blood Glucose Levels at Different Time Points in the 75 g Oral Glucose Tolerance Test and Premature Birth in Pregnant Women with Gestational Diabetes Mellitus

Abstract:Objective To investigate the relationship between abnormal blood glucose levels at different time points of 75 g oral glucose tolerance test(OGTT)and preterm birth in pregnant women with gestational diabetes mellitus(GDM).Methods A total of 400 pregnant women with GDM who received regular prenatal examination and delivered from December 2022 to September 2024 were retrospectively analyzed.All pregnant women underwent 75 g OGTT at 24-28 weeks of gestation.According to the abnormal blood glucose conditions,they were classified as isolated abnormal fasting blood glucose(FBG),isolated abnormal 1-hour post-load blood glucose,isolated abnormal 2-hour post-load blood glucose,and abnormal FBG coupled with abnormal 1-hour or 2-hour post-load blood glucose.The pregnant women were divided into preterm birth group(n=97)and non-preterm birth group(n=303)based on the presence of preterm birth.The clinical data of all pregnant women were collected and the abnormal blood glucose levels at different time points of 75 g OGTT were compared between the two groups.Multivariate logistic regression analysis was used to identify the risk factors for preterm birth in pregnant women with GDM,and receiver operating characteristic(ROC)curve was used to analyze the predictive value of each factor for preterm birth in pregnant women with GDM.Results Compared with the non-preterm group,the preterm group had significantly higher body mass index(BMI)before pregnancy and a significantly higher proportion of family history of diabetes(P<0.01).In the preterm birth group,the incidence of preterm birth in pregnant women with abnormal FBG and abnormal 1-hour or 2-hour post-load blood glucose was significantly higher than that in those with abnormal blood glucose at other time points,and the proportion of abnormal FBG and abnormal 1-hour or 2-hour post-load blood glucose in preterm birth group was significantly higher than that in the non-preterm birth group(P<0.05,P<0.01).Multivariate logistic regression analysis showed that pre-pregnancy BMI,family history of diabetes,abnormal FBG,and abnormal 1-hour or 2-hour post-load blood glucose were independent risk factors for preterm birth in pregnant women with GDM(P<0.05).ROC curve analysis showed that pre-pregnancy BMI,family history of diabetes,abnormal FBG,and abnormal 1-hour or 2-hour post-load blood glucose had certain predictive value for preterm birth in pregnant women with GDM(P<0.01).Conclusions Pre-pregnancy BMI,family history of diabetes,abnormal FBG,and abnormal 1-hour or 2-hour post-load blood glucose are risk factors for preterm birth in pregnant women with GDM,which can significantly increase the risk of preterm birth.Pre-pregnancy BMI,family history of diabetes,abnormal FBG,and abnormal 1-hour or 2-hour post-load blood glucose have predictive value for preterm birth in pregnant women with GDM.
