Prognostic Factors of Papillary Thyroid Microcarcinoma Based on the SEER Database

Abstract:Objective To explore the prognostic factors of patients with papillary thyroid microcarcinoma(PTMC)using large sample data from the SEER database.Methods A total of 55,772 PTMC patients who met the research criteria were selected from the SEER database of the U.S.National Cancer Institute from 2000 to 2019.They were divided into two groups based on whether surgery was performed.Using propensity score matching(PSM),follow-up study subjects were selected from the 55,772 patients at a 3:1 ratio between the surgery group and the non-surgery group.Kaplan-Meier method was used to plot survival curves and calculate overall survival(OS),and the Cox proportional hazards regression model was used to analyze the relationship between surgical treatment and OS in PTMC patients,as well as prognostic factors.Using the PSM method,542 patients in the surgery group and 185 patients in the non-surgery group were finally included as follow-up study subjects.Results Without PSM,there were significant imbalances between the two groups in terms of maximum tumor diameter,age,sex,marital status at diagnosis,income,race,histological subtype,lymph node involvement stage,distant metastasis stage,tumor laterality,and radiotherapy covariates(P<0.05,P<0.01).After matching was completed,no significant differences were observed between the two groups in the above covariates(P>0.05).Kaplan-Meier survival curves showed that,before applying the PSM method,the survival rate of the non-surgery group was lower than that of the surgery group(P<0.01);after applying the PSM method,the same trend remained between the two groups(P<0.01).Multivariate Cox regression analysis showed that:age(≥55 years),male sex,and being unmarried at diagnosis were independent risk factors affecting OS in PTMC patients(P<0.05,P<0.01),and surgery was an independent protective factor(P<0.01).Conclusion Age≥55 years,male sex,and being unmarried at diagnosis are unfavorable to the prognosis of PTMC patients,whereas surgical treatment is more beneficial to survival in PTMC patients.
