Effect of Camrelizumab Combined with Albumin-bound Paclitaxel and Cisplatin Chemotherapy in Different Treatment Courses of Neoadjuvant Chemotherapy on Efficacy and Prognosis of Patients with Esophageal Cancer

Abstract:Objective To investigate the effect of Camrelizumab combined with albumin-bound Paclitaxel and Cisplatin in different courses of neoadjuvant chemotherapy on the efficacy and prognosis of patients with esophageal cancer.Methods Ninety patients with esophageal cancer who received Camrelizumab combined with albumin-bound Paclitaxel and Cisplatin as neoadjuvant chemotherapy from January 2020 to January 2023 were selected.According to the neoadjuvant chemotherapy cycle,patients were divided into the long course group(n=42,3-6 neoadjuvant chemotherapy cycles)and the short course group(n=48,≤2 neoadjuvant chemotherapy cycles).The effect of neoadjuvant chemotherapy was evaluated according to the pathological response during operation.The duration of operation,intraoperative blood loss,postoperative drainage volume,number of lymph node dissected,and total length of hospital stay were collected and recorded.The toxic side effects during treatment and postoperative complications were compared between the two groups.The patients were followed up for 12 to 48 months.Kaplan-Meier survival function was used to compare the survival rate between the two groups.Results The pathological response rate under microscope in the long course group was higher than that in the short course group,and the macroscopic pathological response rate was lower than that in the short course group(P<0.05,P<0.01).The intraoperative blood loss in the long course group was more than that in the short course group,and the total length of hospital stay was longer than that in the short course group(P<0.01).The incidence of toxic side effects and postoperative complications in the long course group was significantly higher than that in the short course group(P<0.05).The overall survival rate of the long course group was 73.81%(31/42),which was significantly higher than that of the short course group[60.42%(29/48)],and the difference was statistically significant(P<0.01).Conclusion The pathological response rate and overall survival rate of patients with esophageal cancer treated with long-course neoadjuvant chemotherapy are higher,but the intraoperative blood loss,length of hospital stay,toxic side effects and postoperative complications are increased.The treatment cycle of neoadjuvant chemotherapy should be considered according to the patient's condition.
