Observation on the Effect of Pelvic Floor Muscle Exercise Combined with External Magnetic Wave Pelvic Floor Therapy for Postpartum Urinary Incontinence

Abstract:Objective To explore the effect of pelvic floor muscle exercise combined with external magnetic wave pelvic floor therapy for postpartum urinary incontinence(PPUI).Methods Sixty-two patients with PPUI admitted from January 2022 to January 2023 were selected and divided into the observation group and the control group according to different treatment regimens.The urinary incontinence at different time points before and after the intervention[Score of the International Consultation on Incontinence Questionnaire-Short Form(ICIQ-SF)],average muscle voltage of type I pelvic floor muscle fibers before and after the intervention,average muscle voltage of type Ⅱ pelvic floor muscle fibers,vaginal dynamic pressure,maximum value of fast muscle contraction,average value of 10-second slow muscle contractions,average value of 60-second slow muscle contraction,maximum urinary flow(Qmax),maximum urethral closure pressure(MUCP),abdominal leakage point pressure(ALPP),connective tissue growth factor(CTGF),osteopontin(OPN),matrix metalloproteinase-1(MMP-1),and quality of life were compared between the two groups.Results At 15 d and 30 d after the intervention and after the intervention,the ICIQ-SF scores of both groups were lower than those at the previous time points,and the scores of the observation group were lower than those of the control group(P<0.05,P<0.01).After the intervention,the average muscle voltage of type I and type Ⅱ pelvic floor muscle fibers and vaginal dynamic pressure in both groups increased,which were higher in the observation group than in the control group(P<0.05,P<0.01).After intervention,the maximum value of fast muscle contraction and the average values of 10-second and 60-second slow muscle contraction in both groups improved to a certain extent,and the improvement effect of the observation group was better than that of the control group(P<0.05,P<0.01).After the intervention,MUCP,ALPP and Qmax in both groups improved to varying degrees,and the improvement in the observation group was more significant than that in the control group(P<0.05,P<0.01).After the intervention,the levels of CTGF,MMP-1 and OPN in both groups decreased to varying degrees compared with those before the intervention,and the decrease was more obvious in the observation group(P<0.05,P<0.01).After the intervention,the scores of each dimension of the comprehensive quality of life assessment questionnaire in both groups increased,and the increase in the scores of each dimension in the observation group was better than that in the control group(P<0.05,P<0.01).Conclusion Pelvic floor muscle exercise combined with extracorporeal magnetic wave pelvic floor therapy can improve the pelvic floor function of patients from both neurological and blood flow aspects,thereby enhancing the therapeutic effect of PPUI and improving the quality of life of patients.
