Clinical Application of Contrast-enhanced Transcranial Doppler Ultrasound in Screening Migraine with Right-to-left Cardiac Shunt

Abstract:Objective To investigate the clinical value of contrast-enhanced transcranial Doppler(c-TCD)in screen-ing migraine with right-to-left cardiac shunt(RLS).Methods A total of 132 patients with migraine admitted from September 2021 to December 2022 were selected as the migraine group,and 132 patients with non-migraine(neuropathic headache)during the same period were selected as the control group.All patients underwent c-TCD.The general information and RLS positive rates,RLS types,and shunt volumes of the two groups were calculated,and the correlation of RLS positive rate,RLS shunt volume with platelet count(PLT)and headache score was analyzed.The positive rate,RLS type and RLS shunt volume of pa-tients with different types of migraine were compared,and the diagnostic value of c-TCD for migraine with RLS was analyzed.Results PLT and headache score in migraine group were higher than those in control group(P<0.01).The positive rate of RLS and the rate of massive shunt in migraine group were higher than those in control group(P<0.01).Spearman correlation analysis showed that RLS positivity and shunt volume were positively correlated with PLT and headache score(P<0.01).The RLS positive rate and the rate of massive shunt in premonitory migraine patients were higher than those in non-premonitory mi-graine patients(P<0.05,P<0.01).The accuracy,specificity and sensitivity of c-TCD in the diagnosis of migraine with RLS were 88.64%,92.45%and 86.08%,respectively.Conclusions c-TCD has a certain diagnostic value in migraine patients with RLS,and the patients have massive shunts,and the RLS positivity and shunt volume are correlated with PLT and headache score.
