Abstract:Objective To investigate the anesthetic effect of ultrasound-guided erector spinae plane block(ESPB)combined with general anesthesia in elderly patients undergoing lumbar surgery. Methods A total of 87 elderly patients undergoing lumbar surgery admitted from January 2022 to June 2024 were selected and divided into the control group(n=44)and the research group(n=43)according to the random number table method. The control group was treated with endotracheal intubation combined with total intravenous-inhalational general anesthesia, and the research group was treated with ultra-sound-guided ESPB combined with general anesthesia. The operation and anesthesia-related indexes, hemodynamic indexes, pain score, sleep quality, analgesic pump use and complication rate were compared between the two groups. Results The dosage of propofol and intraoperative blood loss in the research group were less than those in the control group, the recovery time was shorter than that in the control group, and the duration of operation was longer than that in the control group(P<0.05).During intubation, after intubation and extubation, the mean arterial pressure and heart rate in the research group were lower than those before anesthesia induction and those in the control group(P<0.05).The visual analogue scale(VAS)pain score, pressing times of patient-controlled intravenous analgesia(PCIA)and drug consumption at different time points after operation in the research group were less than those in the control group, and the Pittsburgh sleep quality index(PSQI)score was lower than that in the control group(P<0.05).There was no difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion Ultrasound-guided ESPB combined with general anesthesia can effectively improve the anesthetic effect in lumbar spine surgery in the elderly, reduce the dosage of anesthetic drugs, maintain hemodynamic stability, improve postoperative analgesia and sleep quality, and has high safety.
