Clinical Efficacy of Arthroscopic Release Combined with Intra-articular"Cocktail"Injection in the Treatment of Diabetes Mellitus Complicated with Adhesive Capsulitis of the Shoulder

Abstract:Objective To observe the clinical efficacy of arthroscopic release combined with intra-articular"cocktail"injection in the treatment of diabetes mellitus(DM)complicated with adhesive capsulitis of the shoulder(ACS).Methods The case data of 80 patients with DM complicated with ACS admitted from May 2018 to May 2023 were retrospectively analyzed.All patients were treated with 270 ° joint capsule release surgery under shoulder arthroscopy combined with intra-articular"cocktail"injection.The duration of surgery,length of hospitalization and postoperative complications were recorded.Visual analogue scale(VAS)of shoulder pain,shoulder ranges of motions(ROM),American Shoulder and Elbow Surgeons(ASES)scores,and University of California at Los Angeles(UCLA)shoulder function scores were compared before sur-gery and at various time periods after surgery.Results The duration of surgery and length of hospitalization were(35.23±5.52)min and(3.51±1.01)d respectively,there was 1 patient with shoulder dislocation,and no infection and other complica-tions occurred.The VAS score at each time point from immediately after surgery to 1 week after surgery was reduced com-pared with that before surgery(P<0.05).At 1,3,6,and 12 months after surgery,the ASES and UCLA shoulder function scores of patients with DM complicated with ACS were significantly improved compared with those before surgery(P<0.05).At 3,6,and 12 months after surgery,the ASES and UCLA shoulder function scores as well as shoulder ROM were higher than those at 1 month after surgery(P<0.05).At 6 and 12 months after surgery,shoulder ROM was higher than those at 3 months after surgery(P<0.05).Conclusion Arthroscopic release combined with intra-articular"cocktail"injection has a very good early clinical efficacy in the treatment of DM complicated with ACS.Combination of postoperative rehabilitation exercise can fur-ther consolidate the efficacy,promote the recovery of shoulder function,and maintain a good therapeutic effect in the middle and late stages.
