Early Warning Value of Serum HGF,TARC,and TIMP3 Levels for Refractory Mycoplasma Pneumoniae Pneumonia in Children

Abstract：Objective To investigate the early warning value of serum hepatocyte growth factor(HGF),thymus activation-regulated chemokine(TARC)and tissue inhibitor of metalloproteinase-3(TIMP3)levels in children with refractory Mycoplasma pneumoniae pneumonia(RMPP).Methods One hundred and three children with Mycoplasma pneumoniae pneumonia(MPP)admitted to our hospital from March 2022 to May 2024 were selected and treated with standardized macro-lide antibiotics for 7 d.According to whether they progressed to RMPP,they were divided into RMPP group and non-RMPP group.Clinical data and serum HGF,TARC and TIMP3 levels were compared between the two groups.The relationship be-tween serum HGF,TARC,TIMP3 and the severity of the disease and the early warning value of RMPP were analyzed,and the influencing factors of RMPP were analyzed.Results Of 103 patients,28 patients progressed to RMPP(RMPP group)and 75 did not progressed to RMPP(non-RMPP group).The RMPP group had significantly higher average body temperature,lung involvement area,and disease severity than the non-RMPP group(P<0.01).The RMPP group had significantly higher serum levels of HGF and TARC and a significantly lower level of TIMP3 than the non-RMPP group(P<0.01).The serum levels of HGF and TARC in the non-critically ill,critically ill,and extremely critically ill children showed a gradual increase trend,while TIMP3 showed a gradual decrease trend in the non-critically ill,critically ill,and extremely critically ill children(P<0.05).Spearman correlation analysis showed that serum HGF and TARC were positively correlated with the severity of MPP,and TIMP3 was negatively correlated with the severity of MPP(P<0.01).Multivariate logistic regression analysis showed that serum HGF,TARC,and TIMP3 were independent influencing factors for the risk of RMPP before and after adjusting for other factors(P<0.01).Receiver operating characteristic(ROC)curve analysis showed that the area under the ROC curve(AUC)of serum HGF,TARC and TIMP3 in combination for early warning of RMPP was 0.929(95%CI:0.861,0.970),the sensitivity was 85.71%,and the specificity was 93.33%.The combined early warning value was significantly better than that of each indicator alone(P<0.05).Conclusion Serum HGF,TARC and TIMP3 are significantly correlated with the severity of MPP in children,which are independent influencing factors for the risk of RMPP,and combined detection has higher early warning value.
