Correlative Analysis of Frailty and Lower Extremity Deep Venous Thrombosis in Elderly Male Inpatients with Comorbidities

Abstract:Objective To explore the correlation between frailty and lower extremity deep venous thrombosis(DVT)in elderly male patients with comorbidities.Methods The clinical data of 115 male patients aged 60 years and over admitted to our hospital from September 2022 to June 2023 were collected.With ultrasound diagnosis of lower extremity DVT as the gold standard,the patients were divided into thrombosis group(n=23)and non-thrombosis group(n=92),and the differences in clinical characteristics between the two groups were analyzed.Fried Frailty Scale was used to screen frailty.Univariate logistic regression analysis was used to screen the significant independent variables for the formation of lower extremity DVT,and multi-variate logistic regression analysis was used to identify the independent risk factors for DVT in elderly hospitalized male patients with comorbidities.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of D-dimer(D-D)for lower extremity DVT,and the optimal cut-off value for the diagnosis of DVT was screened according to the Youden index.Results Among 115 hospitalized elderly male patients with comorbidities,23 patients had lower extremity DVT.There were significant differences in age,free triiodothyronine(FT3),C reactive protein(CRP),D-D,homocysteine,urea and N-terminal pro-B-type natriuretic peptide,physical debility,sarcopenia,atherosclerotic cardiovascular disease(ASCVD),proportion of atrial fibrillation and frailty between the thrombosis group and the non-thrombosis group(P<0.05,P<0.01).Multivariate logistic re-gression analysis showed that age,D-D,CRP and frailty were independent risk factors for DVT,and the risk of DVT in patients with frailty was significantly increased(OR=25.472,95%CI:2.820,230.050,P<0.01).The area under the ROC curve of D-D in the diagnosis of DVT was 0.892,and the optimal cut-off value was 1.01 mg/L.Conclusion The prevalence of frailty is high in elderly male patients with comorbidities,and it is highly correlated with DVT.When D-D is higher than 1.01 mg/L combined with frailty,DVT should be considered.
