Observation on the effect of Trastuzumab Combined with Chemotherapy in the Treatment of HER2-positive Metastatic Gastric Cancer Based on Tumor-related Inflammatory Factors and Progression-free Survival

Abstract:Objective To investigate the effect of Trastuzumab combined with chemotherapy in the treatment of human epidermal growth factor receptor 2(HER2)-positive metastatic gastric cancer,as well as its impact on tumor-related inflammatory factors and progression-free survival(PFS).Methods A total of 88 patients with HER2-positive metastatic gastric cancer from March 2022 to March 2023 were selected and divided into the observation group and the control group using simple randomization,with 44 patients in each group.The control group received XELOX chemotherapy(Oxaliplatin+Capecitabine),while the observation group received XELOX chemotherapy combined with Trastuzumab.The therapeutic effects,adverse reactions,and PFS of the two groups were statisti-cally analyzed,as well as the key factors of the interleukin-6/signal transducer and activator of transcription 3(IL-6/STAT3)inflamma-tory signaling axis[IL-6,STAT3,tumor necrosis factor-α(TNF-α)],angiogenesis factors[vascular endothelial growth factor A(VEGFA),matrix metalloproteinase-14(MMP-14)],and tumor markers[carcinoembryonic antigen(CEA),carbohydrate antigen 72-4(CA72-4),carbohydrate antigen 19-9(CA19-9)]before and after treatment.Results The disease control rate in the observation group was 75.00%(33/44),which was higher than that in the control group[52.27%(23/44)](P<0.05).After 2 and 4 cycles of treatment,the lev-els of serum IL-6,STAT3,TNF-α,VEGFA,MMP-14,CEA,CA72-4,and CA19-9 were all lower in the observation group than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).The PFS in the observation group was longer than that in the control group(P<0.05).Conclusion Combination therapy with Trastu-zumab and XELOX chemotherapy can improve the disease control rate in patients with HER2-positive metastatic gastric cancer,inhib-it tumor inflammatory response and neovascularization,prolong PFS,and does not significantly increase adverse reactions.
