Analysis of Misdiagnosis Causes of Cryptogenic Organized Pneumonia Mainly Manifested as Hemoptysis and Literature Review

Abstract:Objective To summarize the clinical characteristics of cryptogenic organizing pneumonia(COP)mainly manifested as hemoptysis and to explore the causes of misdiagnosis.Methods The clinical data of 3 patients with COP mainly man-ifested as hemoptysis who were diagnosed by transbronchial lung biopsy or CT-guided percutaneous lung biopsy from October 2020 to July 2022 were collected,and their clinical manifestations,laboratory tests,imaging findings,pathology and prognosis were compre-hensively analyzed.Results Hemoptysis was the main clinical manifestation in all 3 patients.Chest CT showed consolidation,nod-ules or mass shadows.The initial diagnosis was a right lower lung mass with obstructive pneumonia in 1 patient,a left upper lung mass with possible malignant tumor in 1 patient,and a right lung mass in 1 patient.Organizing pneumonia was confirmed by bronchoscopic biopsy.The misdiagnosis time ranged from 10 d to 10 months.The patient responded well to Glucocorticoid treatment after diagnosis,and the prognosis was of two patients good during follow-up.The lesion of one patient who was readmitted showed no significant ab-sorption.Conclusion COP with hemoptysis is rare and more likely to be missed and misdiagnosed.Strengthening the understanding of clinicians and timely improve the pathological examination can reduce or avoid misdiagnosis.Standardized Glucocorticoid therapy after diagnosis can significantly improving the prognosis of patients.
