Analysis of the Epidemiological Characteristics and Changing Trend of Esophageal Cancer Diagnosed by Gastroscopy

Abstract:Objective To explore the epidemiological characteristics and changing trend of esophageal cancer patients diagnosed by gastroscopy in the past 8 years.Methods A total of 72 689 permanent residents in Kaizhou area who underwent gastroscopy in the People's Hospital of Kaizhou District,Chongqing from January 2016 to December 2023 were selected as the research subjects for retrospective study.The diagnosis of esophageal cancer was confirmed by gastroscopy and pathology.The average annual percentage change(AAPC)was used to compare the trend of changes in esophageal cancer detection rate,and the demographic characteristics and staging characteristics of esophageal cancer patients were analyzed.Results A total of 3 449 patients with esophageal cancer were diagnosed in 72 689 cases undergoing gastroscopy.From 2016 to 2023,485,469,455,440,424,418,389 and 369 patients were detected,respectively.The detection rates were 5.34%,5.16%,5.01%,4.84%,4.67%,4.60%,4.28%and 4.06%,respectively,showing a downward trend year by year,and the AAPC was-2.28%(95%CI:-3.12%,-1.44%),showing significant differences(P<0.01).The AAPC of esophageal cancer de-tection rate was-2.62%(95%CI:-3.91%,-1.32%)among people under 50 years,-2.87%(95%CI:-3.92%,-1.91%)among those aged 50-59 years,-3.12%(95%CI:-5.33%,-1.24%)among those aged 60-69 years,and-3.01%(95%CI:-4.22%,-1.61%)among those aged≥70 years.The detection rate of esophageal cancer in different age groups showed a decreasing trend year by year,and the difference in the changing trend was significant(P<0.01).Of them,34.39%(1 186/3 449)of the esophageal cancer patients were in the early stage,while 65.61%(2 263/3 449)were in the middle and late stage.Conclusion The data of the People's Hospital of Kaizhou District,Chongqing from 2016 to 2023 showed that the detection rate of esophageal cancer showed a downward trend year by year,and most patients were in the middle and late stage when diagnosed.It is necessary to strengthen the health education of esophageal cancer,improve the awareness of esophageal cancer,and take scientific and comprehensive measures to prevent and control it effectively.
