Clinical Effects of Liraglutide Injection Combined with Acarbose and Met-formin in the Treatment of Type 2 Diabetes Mellitus

Abstract:Objective To investigate the clinical effect of Liraglutide injection combined with Acarbose and Metform-in in the treatment of type 2 diabetes mellitus(T2DM)and their effects on islet function,peripheral blood cortisol(Cor),and angiotensin Ⅱ(AngⅡ)levels.Methods A total of 80 patients with T2DM treated from January 2021 to December 2023 were randomly divided into the observation group and the control group,with 40 patients in each group.The control group was treated with Acarbose combined with Metformin,and the observation group was treated with Liraglutide injection on this basis.The clinical efficacy,safety,fasting blood glucose(FBG),2-hour postprandial blood glucose(2 h PG),triacylglycerol,very low density lipoprotein cholesterol(VLDL-C),total cholesterol(TC),homeostasis model assessment of insulin resistance in-dex(HOMA-IR),homeostasis model assessment of β-cell function(HOMA-β),insulin sensitivity index(ISI),microin-flammatory status[NOD-like receptor family protein 3 inflammasome(NLRP3),interleukin(IL)-1β,IL-18]and levels of AngⅡ and Cor in peripheral blood before treatment,and at 3 and 6 months after treatment were compared between the two groups.Results The total effective rate of the observation group[95.00%(38/40)]was higher than that of the control group[77.50%(31/40)],showing significant difference(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Compared with those before treatment,FBG,2 h PG,HbAlc,tria-cylglycerol,VLDL-C,and TC in the two groups at 3 and 6 months after treatment were all decreased,and the decrease in ob-servation group was larger than that in control group(P<0.05).Compared with those before treatment,HOMA-IR,AngⅡand Cor in the two groups were decreased significantly,while HOMA-β and ISI were increased significantly at 3 and 6 months after treatment.The decrease of HOMA-IR,AngⅡ and Cor in observation group was greater than that in control group,and the increase of HOMA-β and ISI in observation group was greater than that in control group(P<0.05).Compared with those before treatment,serum NLRP3,IL-1β and IL-18 in the two groups were significantly decreased at 3 and 6 months after treat-ment,and the decrease in observation group was greater than that in control group(P<0.05).Conclusion Liraglutide in-jection combined with Acarbose and Metformin in the treatment of T2DM can reduce inflammatory reaction,regulate AngⅡand Cor levels in peripheral blood,improve glucose and lipid metabolism,and promote the recovery of islet function.
