Application of New Coagulation Detection Technique in Patients with Early Stage of Traumatic Hemorrhagic Shock Complicated with Traumatic Coag-ulopathy

Abstract:Objective To analyze the clinical value of a new coagulation detection technique(Sonoclot analyzer)in early warning of traumatic hemorrhagic shock(THS)complicated with acute traumatic coagulopathy(ATC).Methods Nes-ted case-control study was used:In the first stage,retrospective case-control study was carried out to analyze the risk factors of early death of THS patients,focusing on the coagulation function index;in the second stage,a prospective study was conduc-ted to explore the efficacy of Sonoclot analyzer in the early warning of ATC in THS patients.Results A total of 215 patients with THS were included in the retrospective study,and followed up to 30 d after injury,of whom 34 patients(15.81%)died.Multivariate logistic regression analysis showed that arterial blood lactate,hemoglobin and prothrombin time were high risk fac-tors for early death in THS patients.The mortality of THS patients with ATC(ATC group)was significantly higher than that of non-ATC group,and the prothrombin time in ATC group was significantly longer than that in non-ATC group(P<0.01).The indexes of coagulation function detected by Sonoclot analyzer included activated coagulation time,coagulation rate and platelet function,and the combined detection with existing coagulation indexes could further improve the prediction efficiency of ATC in THS patients(P<0.05).Conclusion Hyperlactacemia and coagulation dysfunction are high risk factors for the death of THS patients.Sonoclot analyzer can provide early warning of ATC in THS patients,and the combination of existing coagulation indexes can further improve the diagnostic efficiency.
