Analysis of Misdiagnosis in Patients with AIDS Complicated with Dissemi-nated Talaromycosis Marneffei

Abstract:Objective To investigate the clinical features,diagnosis and treatment plans of disseminated Talaromyco-sis marneffei(TSM),and to summarize the causes of misdiagnosis and preventive measures.Methods The clinical data of an AIDS patient with disseminated TSM in 2023 were retrospectively analyzed.Results An AIDS patient presented with fever combined with rash,and abdominal pain combined with lymph node enlargement,anemia,pulmonary disease,hypoproteine-mia,liver insufficiency,which was misdiagnosed as chickenpox and lymphoma,respectively.After admission,pathogenic mi-croorganisms of lymphoid tissue were sequenced and the time of blood culture was prolonged to confirm TSM.Misdiagnosis las-ted more than 1 month.After active and effective antifungal therapy and adjustment of highly active antiretroviral regimen,the patient was cured and the prognosis was good.Conclusion Disseminated TSM can involve multiple organ systems in the whole body,and its clinical manifestations are lack of specificity,which is prone to misdiagnosis or missed diagnosis.Clini-cians should strengthen their understanding of the disease,clarify etiological diagnosis as soon as possible,reduce misdiagno-sis,and improve the success rate of treatment.
