Analysis of Influencing Factors and Construction of Predictive Model for Gastrointestinal Adverse Reactions Related to Immune Checkpoint Inhibi-tors in Patients with Lung Cancer

Abstract:Objective To explore the influencing factors of gastrointestinal adverse reactions related to immune checkpoint inhibitors(ICI)in patients with lung cancer,and to construct a prediction model.Methods A total of 350 pa-tients with lung cancer receiving ICI treatment from July 2020 to July 2022 were randomly divided into the training population(n=245)and the validation population(n=105)according to a ratio of 7∶3 fort a prospective study.The occurrence of ICI-related gastrointestinal adverse reactions in the training population was recorded,and the influencing factors of ICI-related gastrointestinal adverse reactions in lung cancer patients were analyzed through univariate and LASSO-Logistic multivariate re-gression,and the nomogram prediction model was constructed,which was verified in the training and verification population respectively.Results The incidence of ICI-related gastrointestinal adverse reactions in the training population was 46.12%(113/245).Age,drinking history,clinical stage,ICI type,basic history of gastrointestinal tract,history of autoimmune dis-ease,and combined chemotherapy were all factors influencing the occurrence of ICI-related gastrointestinal adverse reactions in lung cancer patients(P<0.01).Based on the above influencing factors,a nomogram prediction model for the risk of ICI-related gastrointestinal adverse reactions in lung cancer patients was established.The C-index of this model in the training and verification population was 0.912 and 0.905,respectively,showing good differentiation and good clinical effectiveness in pre-dicting the risk of ICI-related gastrointestinal adverse reactions in lung cancer patients.Conclusion The influencing factors for the occurrence of ICI-related gastrointestinal adverse reactions in lung cancer patients include age,drinking history,clini-cal stage,ICI type,basic history of gastrointestinal tract,history of autoimmune diseases,and combined chemotherapy.The nomogram prediction model constructed based on the influencing factors is helpful for early clinical screening and identification of high-risk patients.
