Clinical Characteristics of Severe Adenovirus Pneumonia in Children and the Efficacy of Gamma Globulin as an Adjuvant Therapy

Abstract：Objective To explore clinical characteristics of severe adenovirus pneumonia(SAP) in children and the effect of gamma globulin as an adjuvant therapy. Methods A total of 92 children with SAP admitted to De-partment of Paediatrics, Sichuan Provincial People's Hospital between February 2014 and May 2017 who received gamma globulin as an adjuvant therapy were enrolled as gamma globulin treatment group. And 88 children that were not treated with gamma globulin at the same period served as control group. Clinical characteristics of SAP in children were analyzed. In addition,the therapeutic effect, mechanical ventilation time, length of hospital stay, duration of fever,adverse reaction and fatality rate of the two groups were recorded. Results All 180 children with SAP had fe-ver to different degrees,with duration time of 2-25 (12.56 ± 4.15) d. The main manifestations were cough (n=178),dyspnea (n=150) and shortness of breath (n=162). Pulmonary symptoms included wheezing rales in 128 cases,combined respiratory failure in 138,and pleural effusion in 89. A chest X-ray revealed segmental pulmonary consolidation in 94 cases and pleural effusion in 89 cases. The total effective rate was higher in gamma globulin group than in the control group. Duration of mechanical ventilation,length of hospital stay and duration of fever in gamma globulin group were shorter than those of the control group(P<0.05). The total incidence of adverse reactions and mortality rate did not differ significantly between the two groups (P>0.05). Conclusion Children with SAP has high fever,long duration of fever, serious illness and multisystem complications. Gamma globulin can improve the therapeutic effect of SAP in children and shorten the duration of mechanical ventilation,duration of fever and length of hospital stay,which is a safe and reliable approach.
