Subtotal Hysterectomy Due to Uterine Necrosis after B-lynch Suture: a Case Report

Abstract:Objective To explore the cause and preventive measures of subtotal hysterectomy after the B-lynch suture for hemostasis in order to avoid similar situations in the future practice.Methods A retrospective study was performed on a case of subtotal hysterectomy attributed to uterine necrosis in the Second Affiliated Hospital of Army Medical University.Results The patient experienced constant abdominal distension and intermittent vaginal discharge for 4 days after C-section in another hospital.During the C-section,a B-Lynch suture for uterine atony and circular suture technique were performed to control bleeding.After admission to our hospital,the conservative therapy was not well responded and a second abdominal laparotomy was performed.The uterus was found necrotic during surgery and a subtotal hysterectomy plus partial omentum resection was therefore conducted.Then the patient was discharged uneventfully.Conclusion Inappropriate B-lynch suture may cause uterine infection,necrosis or contributes to hysterectomy,which should be weighed cautiously.
