Effects of Laparoscopic Anti-reflux Surgery for Refractory Complex Gastroesophageal Reflux Disease

Abstract:Objective To investigate the safety and effectiveness of laparoscopic anti-reflux surgery (LARS) in the treatment of refractory complex gastroesophageal reflux disease (GERD).Methods Thirty patients with refractory complex GERD admitted to our hospital from January 2012 through December 2016 were enrolled to receive LARS (Nissen,Toupet and DOR fundoplication).The amount of intraoperative blood loss,operation time,perioperative complications,mortality,long-term complications and prognosis were recorded,to gain insight into surgical satisfaction.The quality of life and postoperative follow-up results were evaluated.Results Nissen,Toupet and DOR fundoplication were successfully performed in 18,10 and 2 cases,rcspectively,and the perioperative complication rate was 10%.The average follow-up time was (24.27 ± 1.57) months.The quality of life was evaluated before and after operation.The scores of postoperative symptoms were significantly improved compared with those before operation.The satisfactory rate of surgical treatment was 86.67%.Three cases suffered from chronic dysphagia after operation,and the recurrence occurred in 1 patient,with a recurrence rate of 3.33%.Conclusion LARS is satisfactory,safe and effective in the treatment of refractory complex GERD.
