Analysis of Misdiagnosed Cases of Peripheral Antineutrophil Cytoplasmic Antibody Associated Hypertrophic Cranial Meningitis and Literature Review

Abstract:Objective To explore the clinical characteristics, diagnosis, misdiagnosis reasons and preventive meas-ures for the perinuclear antineutrophil cytoplasmic antibody ( p-ANCA) associated hypertrophic pachymeningitis. Methods A retrospective study was performed for 1 case of p-ANCA associated hypertrophic pachymeningitis which was misdiagnosised as systemic vasculitis and headache, and reviewed the relevant literature. Results In this case, the patient was complaining of pain in the legs gastrocnemius, eye conjunctival congestion for more than 2 years and recurrent headaches for 6 months, with positive p-ANCA. It was be repeated treatment as systemic vasculitis and headache. Eventually, it was diagnosed with p-ANCA associated hypertrophic pachymeningitis after Brain magnetic resonance imaging ( MRI) . The treatment of intrathecal methotrexate and dexamethasone effectively relieved headache symptoms in patients. Conclusion The patients with recurrent headaches should be checked the p-ANCA. Head MRI enhancement scan is helpful for diagnosis of p-ANCA associated hyper-trophic pachymeningitis after excluded the diagnosis of ANCA associated vasculitis. It can decrease or avoid misdiagnosis and mistreatment.
