Misdiagnosis Analysis of 10 Painless Aortic Dissection Patients with Neurological Symptoms as the First Symptom

Abstract:Objective To investigate clinical characteristics, misdiagnosed causes and precautionary measures of aor-tic dissection ( AD) . Methods Clinical data of 10 painless AD patients with neurological symptoms as the first symptom dur-ing January 2015 and May 2017 was retrospectively analyzed. Results The 10 patients all had neurological symptoms as the first symptom, and main manifestations were limbs weakness, dizziness, headache and unconsciousness. There were 6 patients were misdiagnosed as having cerebral infarctions and 4 as having encephalorrhagia at the primary diagnosis. After admission, 8 patients were confirmed by aorta computerized tomography angiography ( CTA) , 1 patient by cardiac ultrasonic cardiogram and 1 patient was found AD during craniocerebral interventional operation. After confirmation, 8 patients quitted therapy;1 patient was transferred to another hospital for surgery and then loss of follow-up;1 patient had good recovery after endovascular treatment. Conclusion Aortic dissections with neurological symptoms as the first symptom is rare, and therefore it is easily be misdiagnosed as cerebrovascular diseases. Careful physical examination, early performing aorta CTA and ultrasonic cardio-gram examinations are beneficial for earlier diagnosis.
