Clinical Analysis of Nutcracker Syndrome Complicated with Chronic Glomerulonephritis

Abstract:Objective To discuss clinical characteristics of nutcracker syndrome (NCS) complicated with chronic glomerulonephritis in order to improve clinician's understanding of the disease.Methods Clinical data of 6 patients with NCS complicated with chronic glomerulonephritis admitted during January 2013 and January 2016 was retrospectively analyzed.Results There were 4 males and 2 females, and mean age was (22.33±6.72) years old.The first symptom was hematuria in 3 patients, lumbago in 2 patients and proteinuria in 1 patient respectively.All patients had proteinuria, and 5 had hematuria simultaneouly in the course of the disease.NCS was confirmed by Doppler sonography, and further renal biopsy was performed to confirming the diagnosis of combination of chronic glomerulonephritis.Among the patients, there were 4 patients with IgA nephropathy (LeeⅡ, LeeⅢ, LeeⅢ-Ⅳ, LeeⅣ respectively), 1 with membranous nephropathy and 1 with minimal change of the glomerular respectively.The 2 patients combined with IgA nephropathy were given glucocorticoid and Mycophenolate Mofetil treatments, and the left 5 patients were given conservative treatment.Tonsillectomy was performed in 1 patient with IgA nephropathy and tonsillar enlargement.Conclusion NCS can exist alone, and it also can combine with chronic glomerulonephritis, and therefore NCS patients combined with hematuria and (or) proteinuria should do renal biopsy to further confirm whether it combines with chronic glomerulonephritis or not, and it can help to prevent misdiagnosis and missed diagnosis.
