Obsolete Uterine Perforation Complicated with Fallopian Tube Ectopia: a Case Report and Literature Review

Abstract:Objective To discuss key points of clinical diagnosis and treatment for obsolete uterine perforation complicated with fallopian tube ectopia.Methods Clinical data of one patient with obsolete uterine perforation complicated with fallopian tube ectopia was retrospectively analyzed,and relevant literature was reviewed.Results The patient was admitted for decreased menstrual blood volume after curettage for 16 years and left hydrosalpinx for more than 2 months.Curettage was performed due to placental adherence and postpartum hemorrhagic shock in past production,and the menstrual cycle recovered after the surgery,but the menstrual blood volume gradually decreased.Uterine fallopian tube of lipiodol angiography showed left hydrosalpinx,right fallopian tube atresia and endometrial continuity interrupt.Transvaginal ultrasound examination showed irregular uterine shape,uneven echo of uterus wall,0.78 cm endometrial thickness and slightly uneven signal of endometrium.The patients was confirmed obsolete uterine perforation complicated with fallopian tube ectopia by hysteroscopy combined with laparoscopy,and then fallopian tube reset and uterine repair were performed,and then postoperative antibiotics therapy was given for preventing infection.The patient received Estradiol Tablets orally for regulating endometrium after discharge.In postoperative 2 months,hysteroscopy was performed again,and it showed acceptablely intrauterine shape and volume,uniform intima and good recovery at the point of perforation repair.The patient had no comfortlessess by telephone follow-up in postoperative 6 months.Conclusion Obsolete uterine perforation complicated with fallopian tube ectopia lacks of specific manifestations,and therefore postpartum patients undergoing curettage should be given comprehensive examination in order to early diagnosis and treatment to improve prognosis of patients.
