Clinical Characteristics and Misdiagnosed Causes of Patients with Cryptococcus Neoformans Meningitis

Abstract:Objective To investigate clinical characteristics and misdiagnosed causes of cryptococcus neoformans meningitis( CNM)in order to reduce the misdiagnosis rate. Methods Clinical data of 12 misdiagnosed patients with CNM admitted during January 1995 and January 2016 was retrospectively analyzed. Results The first symptoms were headache, dizziness,vomiting and fever mainly. A total of 11 patients were diagnosed and treated in other hospitals. The 8 patients were misdiagnosed,in which 4 patients were misdiagnosed as having upper respiratory tract infection and 3 patients as having tuber-culous meningitis in other hospitals,and 1 patient was misdiagnosed as having bacterial meningitis in our hospital,and misdi-agnosed rate was 75. 00﹪. CNM was confirmed after repeatedly cerebrospinal fluid ink staining and cytopathology examina-tions,and 2 patients were cured,and 10 patients turned for the better after anti-fungal treatment. Conclusion Patients with CNM are mainly young and sporadic type. Clinicians should repeatedly perform cerebrospinal fluid ink staining and cytopathol-ogy examinations to avoid misdiagnosis.
