Surgical Timing and Prognostic Influence of Thoracic Spinal Tuberculosis Complicated with Spinal Cord Injury

Abstract:Objective To investigate surgical timing and prognostic influence of thoracic spinal tuberculosis compli-cated with spinal cord injury. Methods A total of 138 patients with thoracic spinal tuberculosis complicated with spinal cord injury during January 2013 and December 2015 were divided into observation group (n=70, over 4 weeks) and control group ( n=68, less than 4 weeks) according to the standard anti-tuberculosis treatment time before operation. Intraoperative bleed-ing volume, surgical duration, fusion time of bone graft, changes of erythrocyte sedimentation rate ( ESR) before and after surgery, postoperative condition of pain relief [ visual analogue scale ( VAS) score] , neurological function improvement score [ American spinal injury association score ( ASIA) ] and postoperative complications were compared between the two groups. Results All operations were successfully completed, and there were no statistically significant differences in intraoperative bleeding volume, surgical duration and fusion time of bone graft between the two groups (P>0. 05). All patients were fol-lowed up for 12-48 months. There were 65 patients with primary healing and 5 patients with delayed healing and sinus tract formation in observation group, while there were 66 patients with primary healing and 2 patients with recurrence in control group. Postoperative ESR and VAS scores were significantly decreased compared with those before operation in each group (P<0. 05), but there were no statistically significant differences between the two groups (P>0. 05). ASIA scores were sig-nificantly improved compared with those before operation in each group, and the improvement was obvious in observation group, and there were statistical differences between the two groups (P<0. 05). Conclusion Timely surgery on the basis of standard anti-tuberculosis treatment before operation in treatment of thoracic spinal tuberculosis complicated with spinal cord injury can help to improve standard anti-tuberculosis treatment.
