Effect of Short-term Intensive Insulin Therapy with Insulin Pump on the Level of Oxidative Stress in Patients with Type 2 Diabetic Lower Extremity Arterial Disease

Abstract:Objective To investigate the effect of short-term intensive insulin therapy with insulin pump in treatment of the low limb vascular disease (LEAD) patients with type 2 diabetes mellitus (T2DM) and explore the possible mechanism. Methods A total of 80 LEAD patients with T2DM between January 2012 and January 2015 were randomly divided into two groups:patients with intensive insulin therapy with insulin pump (intensive group, n=40) and patients without intensive in-sulin therapy ( unintensive group, n=40 ) . Intensive group underwent intensive Insulin therapy for 14 d, unintensive group was given Insulin injection twice a day for 14 d. The levels of fasting plasma glucose ( FPG ) , 2 h postprandial glucose ( 2 hPG) , homocysteine ( Hcy) , von Willebrand factor ( vWF) and ischemia-modified albumin ( IMA) before and after treat-ment were tested in the two groups, and the indexes were compared and analyzed. Results The levels of FPG, 2 hPG, Hcy, vWF and IMA in the two groups were decreased compared with those before treatment(P<0. 01 , P <0. 05), levels of FPG, 2 hPG, Hcy, vWF and IMA in intensive group were obviously decreased compared with those of unintensive group ( P<0. 05). Conclusion Short-term Insulin therapy with Insulin pump in patients with diabetes mellitus may improve the level of oxidative stress and vascular endothelial function, and therefore it can delay the development of LAED.
