Prognostic Influence of Differential Pressure of Centra Venous-arterial Carbon Dioxide on the Patients with Severe Pneumonia

Abstract:Objective To explore the prognostic influence of differential pressure of centra venous -arterial carbon di-oxide ( Pcv-aCO2 ) on patients of severe pneumonia .Methods This study selects 79 patients with severe pneumonia in the department of respiratory diseases , the First Hospital of Shijiazhuang , and the levels of Pcv-aCO2 were detected upon admis-sion in 6 hours, 12 hours and 24 hours.The patients were divided into Pcv-aCO2 <6.0 mmHg group (group A), Pcv-aCO2 6.0-10.0 mmHg group (group B) and Pcv-aCO2≥10.0 mmHg group (group C) in accordance with initial Pcv-aCO2 aver-age, and gender , age, complications , duration of mechanical ventilation , accurate physiology and chronic health evaluation II (APACHE II) scores, 28 d mortality rate were compared among the three groups .These groups were divided into survival group and mortality group based on 28 d disease outcome , and changes of Pcv-aCO2 level in survival group and mortality group in different hours were analyzed .Results Complication , duration of mechanical ventilation , APACHE II scores and 28 d mortality rate between the different levels of Pcv-aCO2 of three groups were statistically significant ( P<0.05 ) .Complication rate of different levels of Pcv-aCO2 in group A and group B were lower than that in group C; APACHE II score, duration of mechanical ventilation and 28 d mortality rate in group A were lower than that of group B and group C , while group B was low-er than group C and the differences were statistically significant ( P<0.05 ) .In 28 days after admission , 55 of 79 patients sur-vived and 24 patients died in this study .The average level of Pcv-aCO2 in survival group in different hours was lower than those in the mortality group , and differences were statistically significant ( P<0.05 );compared with that at admission hour , the level of Pcv-aCO2 of survival group on the 24th hour declined, and differences were statistically significant (P<0.05). Conclusion The level of Pcv-aCO2 can be taken as an index to judge the severity and prognosis of patients with severe pneu-monia , which can reflect the blood perfusion of patients with severe pneumonia .The higher the level is , the worse the progno-sis is.
