Transurethral Plasmakinetic Pole-electrode En Bloc Enucleation for the Treatment of Non-muscle-invasive Bladder Cancer

Abstract:Objective To study the feasibility and clinical effects of transurethral plasmakinetic pole-electrode en bloc enucleation for non-muscle-invasive bladder cancer. Methods We enrolled in the study 33 patients with non-muscle-in-vasive bladder cancer confirmed by cystoscopy and CT. The patients received transurethral plasmakinetic pole-electrode en bloc enucleation under epidural anesthesia. After the tumor tissues were cleared away, 100 ml of distilled water were perfused into bladder for 5 min. The first bladder perfusion with Pirarubicin within 6 hours and then succedent routine bladder perfusion was followed. The operative time, blood loss, indwelling urethral catheter time and complications were observed. Results The patients in our study included 23 males and 10 females aged 32-74. The tumor diameter was 0. 8 cm to 2. 5 cm and the number of tumor was 1 to 3. All the bladder tumors were en bloc enucleated with the operation time of 5-25 min per tumor. There were 2 minor obturator nerve reflexes and no bladder perforation occurred. Peri-operative blood loss was minimal. Post-operative indwelling urethral catheter time was 1-3 d. After a follow-up for 3-18 m, one patient with multitumor had recurrence but was cured after transurethral operation. Conclusion Transurethral plasmakinetic pole-electrode en bloc enucleation in the treatment of non-muscle-invasive bladder cancer is feasible and safe with good prognosis.
