Cause Analysis for Delayed Diagnosis and Difficulty in Treatment of Spinal Tuberculosis

Abstract:Objective To explore the clinical characteristics of spinal tuberculosis patients in order to improve the di-agnosis and treatment of the spinal tuberculosis. Methods The clinical data of 50 spinal tuberculosis patients admitted to Changsha Central Hospital for treatment were collected and retrospectively analyzed. Results Spinal tuberculosis frequently occurred in young and middle aged adults in this group (72%);35 cases (96%) were secondary to tuberculosis;the lesions were found mainly in lumbar spines;28 cases (56%)and thoracolumbar spines;10 cases (20%);23 cases (46%) were re-treatment cases;the major clinical manifestations were local pain and limited activity; 32 spinal tuberculosis patients were misdiagnosed at the first visit, and the misdiagnosis rate was 64%. Spinal tuberculosis was confirmed according to symptoms and signs, tuberculin purified protein derivative skin test, serum tuberculosis antibody detection, imaging examination, wheth-er with tuberculosis on the other parts of the body or not, paravertebral abscess puncture, laboratory examinations, surgical bi-opsy, the effective treatment of diagnostic anti tuberculosis. 35 patients (70%) were cured. Conclusion Spinal tuberculosis is prone to be misdiagnosed and the treatment is difficult. Complicated symptoms and large numbers of patients for retreatment are main causes of misdiagnosis and difficulty in treatment. The key to improve treatment effect is to raise awareness of the dis-ease and apply treatment strategy as soon as possible.
