Electrocardiogram Characteristics of Acute Inferior Myocardial Infarction Caused by Occlusion of Left Circumflex Artery

Abstract:Objective To investigate electrocardiogram ( ECG) characteristics of acute inferior myocardial infarction ( AIMI) attributing to occlusion of left circumflex artery ( LCX) and predictive value of ST-segment changes on infarct-related&nbsp;artery ( IRA) . Methods We retrospectively analyzed the clinical characteristics, electrocardiogram and coronary angiography of 240 patients with AIMI who underwent primary percutaneous coronary intervention ( PCI) within 12 hours of symptom onset. Results The IRAs were the right coronary artery (RCA) in 177 patients (73. 8%) and the LCX in 63 cases (26. 2%). There were no significant differences in the patients' demographics and risk factors between the two groups. Significant differ-ences were found in ST segment deviation of lead Ⅰ, Ⅲ, Ⅲ-Ⅱ, aVL, aVF, aVL-I, V1 , V6 between both groups ( P<0. 01). According to Receiver Operating Characteristic curve, ST-segment depression (STD) in lead I, ST-segment elevation ( STE) in lead Ⅲ<Ⅱ and STD in lead all < I suggested the IRA of AIMI was LCX, and the diagnostic method had higher sensitivity and specificity. Conclusion STD in lead I, STE in lead Ⅲ<Ⅱand STD in lead all < I have certain predictive value in estimating LCX occlusion as IRA of AIMI.
