Acute Pulmonary Thromboembolism Due to Tricyclic Antidepressant:a Case Report and Literature Review

Abstract:Objective Doxepin is a tricyclic antidepressant which is also a serotonin norepinephrine reuptake inhibi-tor used for the management of major depression. Objective:to reduce missed diagnosis and misdiagnosis rates of acute pulmo-nary thromboembolism which may be caused by Tricyclic antidepressants. Methods Clinical data of a case of acute pulmona-ry thromboembolic event due to long-time ingestion of doxepin was retrospectively analyzed and the literature review was made. Results The patient was a 71-year-old female, admitted for dizziness for 2 h and a history of anxiety disorder for 5 years. Upon admission, the patient was given oxygen therapy and medication to improve blood circulation and control blood pressure. The patient was finally diagnosed as having acute pulmonary embolism and her condition improved after undergoing ant conges-tion therapy. The patient stopped medication upon discharge and was followed up for 9 months. No reoccurrence of pulmonary embolism was reported. Conclusion We need to pay more attention to the risk of thromboembolic events with patients of syn-cope and oral antidepressant treatment. PE should be considered. TEG ( thromboembolic-storm TEG) may be helpful in analy-zing pathological mechanisms of thrombogenesis and planning anticoagulant therapy.
