A Correlation Study Serum Bilirubin Level and Early Stage Diabetic Nephropathy

Abstract:Objective To study the relationship between the levels of serum bilirubin and early stage diabetic ne-phropathy and influencing factor on diabetic nephropathy. Methods 102 patients with type 2 diabetes mellitus were divided into diabetes mellitus group(n=48) and diabetic nephropathy group(n=54) according to urine albumin urinary albumin ex-cretion rate ( UAER) . Serum total bilirubin, direct bilirubin, body mass index, blood pressure, ALT, lipid profiles, fasting plasma glucose, HbA1c and insulin were measured and analyzed. Results The course of disease in diabetes mellitus group was significantly longer than that in diabetic nephropathy group. The level of serum bilirubin in diabetes mellitus group was significantly higher than those in diabetic nephropathy group(P<0. 05). The level of serum bilirubin was negatively associat-ed with UAER (r= -0. 261, P<0. 05). Logistic regression analysis showed that course of the disease, LDL-c, and TBil, SBP were independent risk factors of diabetic nephropathy (OR=1. 108、2. 593、0. 824、1. 069;P<0. 01, P<0. 05). Con-clusion High serum bilirubin can play a protective role in the development of DN. Close monitoring of TBil, early interven-tion and control of LDL-c and SBP can reduce incidence rate of diabetic nephropathy.
