A Case Report and Literature Review:Intravenous Leiomyomatosis Misdiagnosed as Epilepsy

Abstract:Objective To enhance the understanding of clinical characteristics of intravenous leiomyomatosis (IVL) to a-void the misdiagnosis and mistreatment. Methods Clinical data of a patient with intravenous leiomyomatosis was retrospectively an-alyzed, and related literature was reviewed. Results The patient was admitted for paroxysmal unconsciousness for 4 months. He was misdiagnosed as having epilepsy in a local hospital. But the therapeutic effect was unsatisfactory after related treatment. The pa-tient was confirmed as having intravenous leiomyomatosis after the color Doppler ultrasound examination of lower limbs, department of gynaecology, inferior vena cava and hepatic vein. CTPA and CTV were also performed. The patient underwent surgical treatment under general anesthesia. IVL, leiomyoma of uterus, IVL of uterus were confirmed and cor dextrum system and pulmonary artery were affected after postoperative pathologic examination. The patient was discharged from hospital after improvement. Conclusion To avoid misdiagnosis, clinicians should enhance awareness of intravenous leiomyomatosis, carefully ask of medical history and ad-vise physical check-ups, and perform related imageological examinations, which are the key to avoid misdiagnosis.
