Correlations between serum 25-OH-vitamin D and diabetic foot ulcer in diabetic patients

Abstract：Objective To assess the association between levels of 25-hydroxy vitamin D(25OHD)and diabetic foot ulcers(DFU)so as to provide clinical evidence for the prevention and treatment of DFU.Methods A total of 586 patients with diabetes mellitus(DM)admitted to the Department of Endocrinology of the Air Force Medical Center between January 2020 and March 2022 were selected as the subjects,who were divided into the DFU group(n=318)and the DM group(n=268).The patients'age,courses ofdisease,gender,smoking history,drinking history,25OHD levels,and blood biochemistry were recorded before the differences between the DFU group and the DM group were analyzed.Multivariate logistic regression was used to analyze the correlations between 25OHD and DFU,and the changes of 25OHD levels in DFU patients with different Wagner grades were observed.Results There were statistically significant differences in age,duration of diabetes,systolic blood pressure,diastolic blood pressure,BMI,serum creatinine,blood urea nitrogen,total cholesterol,triglyceride,high density lipoprotein cholesterol,low density lipoprotein cholesterol,alanine aminotransferase,aspartate aminotransferase,albumin,white blood cell count,hemoglobin,hematocrit,and 25OHD levels between the two groups(t/Z=4.845-144.330,all P＜0.050).Multivariate logistic regression analysis showed that 25OHD was an independent protective factor for DFU(OR=0.984,95％CI:0.971-0.997,P=0.015).After adjusting for all potential confounders,25OHD remained an independent protective factor for DFU(OR=0.984,95％CI:0.971-0.997,P=0.018).The rate of 25OHD deficiency in the DFU group was higher than in the DM group(x2=16.653,P＜0.05).The level of 25OHD in Wagner 1-5 DFU patients trended down(P＜0.010).Conclusion Ahigh level of 25OHD is an independent protective factor for DFU,and vitamin D supplementation may help prevent the occurrence of DFU and improve its recovery.
