Mammography, dynamic enhanced MRI and diffusion weighted sequence in diagnosis of breast ductal caicinoma in situ and infiltrating carcinoma

Abstract:Objective To explore the diagnostic value of mammography,dynamic enhanced MRI and diffusion weighted sequence in diagnosis of breast ductal caicinoma in situ and infiltrating carcinoma.Methods We collected 67 cases of female patients with breast lumps between June 2013 and June 2017 in our hospital.They had respectively received molybdenum target X-ray digital imaging,dynamic enhanced MRI scan(CE-MRI)and diffusion weighted imaging(DWI) preoperatively.The diagnostic value of the three inspection methods was evaluated pathologically.Results In this study,pathology confirmed 43 cases of malignant tumors and molybdenum target X-ray diagnosed 32 of them as malignant lesions,so the sensitivity was 74.42%(32/43).By contrast,CE-MRI diagnosed 39 of these cases as malignant lesions,so the sensitivity was 90.70%(39/43),while DWI diagnosed 40 of these cases as malignant lesions,and the sensitivity was 93.02% (40/43).There was statistically significant difference between the three approaches (P<0.05) .Of the 24 cases of benign cases that were pathologically proven,17 cases were diagnosed as benign by molybdenum target X-ray,so the specificity was 70.83%(17/24),compared to 19 benign cases and a specificity of 79.17% with CE-MRI(19/24),and 21 benign cases and a specificity of 87.50%(21/24)with DWI.There was no statistically significant difference (P>0.05) in this regard.Pathology confirmed 43 cases malignant tumors and 24 cases of benign cases.Molybdenum target X-ray diagnosed 32 cases as malignant lesions,and 17 cases as benign,so the rate of accuracy was 73.13%(49/67).CE-MRI diagnosed 39 cases of malignant lesions and 19 cases of benign ones,so the rate of accuracy was 86.57%(58/67).DWI diagnosed 40 cases of malignant lesions and 21 cases of benign ones,so the accuracy was 91.04%(61/67),suggesting statistically significant difference(P<0.05).Conclusion CE-MRI and DWI have a higher detection rate and diagnostic accuracy than X-ray imaging in ductal carcinoma in situ and invasive carcinoma of the breast.
