Predictive value of ABCD3-Ⅰ score combined with transcranial Doppler in patients with secondary cerebral infarction after transient ischemic attack

Abstract：Objective To investigate the predictive value of ABCD3-Ⅰ score combined with transcranial Doppler (TCD)in secondary cerebral infarction(CI)within 7 days of transient ischemic attack(TIA).Methods Sixty-three patients with TIA treated in our hospital between January 2015 and December 2016 were selected,whose imaging data and clinical data were collected.These patients were evaluated by ABCD3-Ⅰ scoring method combined with transcranial Doppler data.The incidence of cerebral infarction among these patients within 7d was calculated.Results Of the 63 patients with TIA,16 were at low risk(25.39%),35 were at risk(55.55%),and 12(19.04%)were at high risk(ABCD3-Ⅰ).The incidence of cerebral infarction in the high-risk group was 41.66%,which was significantly lower than that in the low-risk group(6.25%)and the middle risk group(22.85%),and the difference was statistically significant(P＜0.05)(7 d).The incidence of cerebral infarction in normal or mild,moderate and high cerebral artery stenosis was 8.69%,13.63% and 50.00% respectively.The incidence of cerebral infarction in highly stenotic patients within 7 d was significantly higher than that of patients with normal or mild or moderate stenosis(P＜0.05).TIA patients whose cerebral artery stenosis exceeded 50% and ABCD3-Ⅰ score was 4 or above were more likely to die from secondary cerebral infarction within 7 d than those with mild symptoms.Conclusion ABCD3-Ⅰ scoring combined with transcranial Doppler can effectively improve the accuracy of predicting secondary cerebral infarction within 7 days of transient ischemic attack.
