Clinical efifcacy of noninvasive positive pressure ventilation in the treatment of aged patients with consciousness disturbance due to AECOPD and typeⅡ of respiratory failure

Abstract:ObjectiveTo investigate the clinical efficacy of noninvasive positive pressure ventilation in the treatment of aged patients with consciousness disorders due to chronic obstructive pulmonary disease in the acute stage (AECOPD) and typeⅡ respiratory failure.Methods43 elderly patients with consciousness disorders due to AECOPD complicated with typeⅡ respiratory failure were selected in the study. All the patients were above 65. They received routine therapies plus noninvasive positive pressure ventilation (VENTImotion, Weinmann Germany). Meanwhile,the change of heart rate, respiratory rate arterial blood gas indexes and conscious state, were observed before and after treatment.ResultsAll the patients were treated effectively. The pH value and pressure of arterial carbon dioxide (PaCO2) were significantly improved 2 h after treatment (P<0.05). The patients' state of consciousness was improved, the average Glasgow coma scale (GCS) was increased (P<0.01), their heart rate and respiratory rate decreased and PaO2 increased markedly 12 hours after treatment (P<0.05). The arterial blood gas indexes almost returned to normal 48 h after treatment.ConclusionAuxiliary noninvasive positive pressure ventilation treatment is quite effective for senile chronic obstructive pulmonary disease in the acute stage with typeⅡ of respiratory failure associated with disturbance of consciousness.
