The Application of Nasal Humidified High-flow Pre-oxygenation During the Induction Period of General Anesthesia in Patients

Abstract:Objective To observe the effect of nasal humidification high-flow pre-oxygenation technique in the induction period of general anesthesia patients.Methods Fifty patients scheduled for general anesthesia surgeries from November 2024 to April 2025 were selected.There were 29 males and 21 females,aged 18 to 60 years old,whose ASA classification was Ⅰ or Ⅱ,Mallampatti classification was Ⅰ or Ⅱ,and BMI ranged from 18 to 28 kg/m2.They were randomly divided into two groups:Group A(mask pre-oxygenation)and Group B(nasal humidification high-flow pre-oxygenation),with 25 patients in each group.The induction methods of the anesthetic drugs were the same for both groups.Before induction,Group A received pre-oxygenation with a mask for 5 minutes,while Group B was pre-oxygenated with an oxygen flow of 50 L/min through a nasal catheter for 5 minutes after warming and humidification.The MAP,HR,and blood gas analysis(PaO2,PaCO2,FiO2/PaO2)were recorded at each time point:at the time of entering the operating room(T0),5 minutes after pre-oxygenation(T1),before tracheal intubation(T2),and after tracheal intubation(T3).The safe asphyxia time of the patients in both groups,PetCO2 immediately after tracheal intubation,whether the patients in both groups experienced a sharp drop in SpO2 requiring mask-assisted pressurized oxygenation,and the adverse reactions of the patients were recorded.Results At T3,the MAP and HR of group A were significantly higher than those at T0 and T2(P<0.05),and compared with group B,the differences were statistically significant(P<0.05);at T1,T2,and T3,the PaO2 of both groups increased(P<0.05).At T1,the PaO2 of group B was significantly higher than that of group A(P<0.05),and the increase value of PaO2(T0-1)was significantly higher than that of group A(P<0.05);at T2,the increase value of PaO2(T0-2)was significantly higher than that of group A(P<0.05).The safe asphyxia time of group B was significantly longer than that of group A(P<0.05).In group A,6 cases(24.00%)required mask-assisted pressurized oxygen supply,and there was a statistically significant difference compared with group B(P<0.05).Conclusion During the induction period of anesthesia,high-flow nasal humidification pre-oxygenation can increase PaO2,prolong the safe asphyxia time,stabilize hemodynamic indicators,and have fewer adverse reactions.It can be safely applied.
