The Effect of Phacoemulsification Combined with Goniosynechialysis on Angle-closure Glaucoma Complicated with Cataract

Abstract:Objective To explore and analyze the impact of phacoemulsification combined with primary posterior chamber intraocular lens implantation and goniosynechialysis on the treatment of angle-closure glaucoma combined with cataract.Methods Patients diagnosed with primary angle-closure glaucoma and cataract at the Department of Ophthalmology,Gao'an People's Hospital,Jiangxi Province,from January 2021 to December 2023 were selected as the study subjects.The research subjects were randomly grouped through computer programming.In the end,92 patients were enrolled.The observation group received phacoemulsification and aspiration of cataract combined with one-stage implantation of posterior chamber intraocular lens and angle separation surgery(n=46),while the control group received phacoemulsification and aspiration of cataract combined with one-stage implantation of posterior chamber intraocular lens and trabeculectomy(n=46).Statistical analysis was conducted to determine whether there were differences in indicators such as best corrected visual acuity(BCVA),intraocular pressure,angle opening status,and central anterior chamber depth one month after the operation.Results One month after the operation,both groups were re-examined:The best corrected visual acuity(BCVA)of the observation group was significantly higher than that of the control group(P<0.01).The intraocular pressure of both groups was significantly lower after the operation than before,but there was no statistically significant difference in the degree of pressure reduction between the two groups(P>0.05).The central anterior chamber depth of the observation group was significantly increased compared with that of the control group after the operation(P<0.05),and the angle opening of the observation group was significantly wider than that of the control group after the operation(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).Conclusion For patients with primary angle-closure glaucoma combined with cataract,treatment with phacoemulsification combined with primary intraocular lens implantation and goniosynechialysis yields clinical outcomes that align more closely with expectations.This approach significantly reduces the probability of complications during treatment,markedly lowers postoperative intraocular pressure,significantly deepens the central anterior chamber,further opens the angle,and improves BCVA.The clinical efficacy is definite,with high safety,making it worthy of further clinical promotion and application.
