Effect of Extracorporeal Diaphragm Pacing Combined with Rehabilitation Treadmill in Patients with Stroke and Invasive Mechanical Ventilation

Abstract:Objective To investigate the effect of extracorporeal diaphragm pacing therapy combined with bed rehabilitation treadmill exercise in patients with stroke and invasive mechanical ventilation.Methods 60 patients who were admitted to the intensive care unit of the First Affiliated Hospital of Jinzhou Medical University from September 2023 to May 2024 and had cerebral stroke as the primary disease and received invasive ventilator ventilation treatment were selected.All patients were divided into the observation group(30 cases)and the control group(30 cases)according to the random number table method.Both groups of patients received conventional treatment and performed upper limb active or passive rehabilitation cycling exercises on the bed.The observation group was additionally treated with extracorporeal diaphragm pacing low-frequency electrical stimulation on the basis of the control group.Results The diaphragm activity,oxygenation index(PaO2/FiO2,P/F),cough peak expiratory flow(CPF),maximal inspiratory pressure(MIP),and the success rate of weaning from the ventilator in the control group were all lower than those in the observation group,and the differences were statistically significant(P<0.05).The rapid shallow breathing index(RSBI),ventilator ventilation time,and length of stay in the intensive care unit in the control group were all greater than those in the observation group,and the differences were also statistically significant(P<0.05).Conclusion Extracorporeal diaphragm pacing therapy combined with bed-based rehabilitation cycling exercises can improve the oxygenation effect,diaphragm muscle strength,airway clearance ability and weaning success rate of patients,while also reducing the shallow and rapid breathing index,shortening the ventilator ventilation time and the length of stay in the intensive care unit.
