The Application of Lateral Lying at Different Angles Combined with Lung Recruitment in Patients with Moderate to Severe Acute Respiratory Distress Syndrome

Abstract:Objective To explore the efficacy of lateral ventilation at different angles combined with pulmonary supplementation in the treatment of moderate to severe acute respiratory distress syndrome(ARDS).Methods 60 patients with moderate to severe ARDS who were hospitalized in the Intensive Care Unit of the First Affiliated Hospital of Jinzhou Medical University and received invasive mechanical ventilation treatment from June 2022 to February 2023 were selected.The control group adopted the 30° turning and supine position,while the observation group adopted the 90° turning and supine position.Both groups of patients used invasive ventilator-assisted ventilation and received lung recruitment treatment twice a day.Through comparison,it was found that there was no statistically significant difference in the personal basic data of the two groups of patients.The differences in relevant indicators of respiratory mechanics and oxygenation effect between the two groups of patients when using different lateral turning angles on the premise that both groups adopted lateral turning once every 2 hours,as well as the differences in the duration of invasive mechanical ventilation,the duration of stay in the intensive care unit,and the 28-day mortality rate were compared.Results The relevant indicators of arterial blood gas analysis,oxygenation index(PaO2/FiO2,P/F),functional residual capacity(FRC)of the lung,and lung compliance in the control group were all lower than those in the observation group.The ventilation time of invasive ventilator,the length of stay in the intensive care unit,and the 28-day mortality rate in the control group were higher than those in the observation group.Conclusion The 90° lateral position ventilation combined with invasive ventilator ventilation lung recruitment therapy for the treatment of patients with moderate to severe ARDS can improve oxygenation effect and respiratory mechanics-related indicators,and at the same time can reduce the duration of invasive ventilator ventilation,the length of stay in the intensive care unit,and the 28-day mortality rate.
