Evaluation of Left Ventricular Function and Correlation with Plasma Homocysteine Levels in Patients with Type 2 Diabetes Mellitus by Automated Cardiac Motion Quantitation

Abstract:Objective To evaluate left ventricular function in patients with type 2 diabetes mellitus with or without elevated homocysteine(Hcy)by applying automated cardiac motion quantification(aCMQ),and to compare and analyze the correlation between the presence or absence of elevated homocysteine on left ventricular systolic and diastolic function in patients with T2DM.Methods A total of 100 T2DM patients(EF>55%)with unimpaired left ventricular ejection fraction were enrolled in this study and were divided into the T2DM combined with hHcy group and the T2DM alone group on the basis of plasma homocysteine levels.Another 50 age-and sex-matched healthy volunteers were recruited as the control group.ACMQ was used to measure left ventricular apical two-chamber longitudinal strain(AP2LS),apical three-chamber longitudinal strain(AP3LS),apical four-chamber longitudinal strain(AP4LS),left ventricular global longitudinal strain(LVGLS),and left ventricular shor taxis basal segment circumferential strain(SAXBCS),left ventricular shor taxis middle circumferential strain(SAXMCS),left ventricular shor taxis apical circumferential strain(SAXACS)and left ventricalar global circumferential strain(LVGCS).The correlation between Hcy and aCMQ parameters was analyzed,and the risk factors of early cardiac injury and the corresponding diagnostic value were determined by linear regression and area under subject operating characteristic curve(AUC).Results AP2LS,AP3LS,AP4LS and LVGLS were lower in patients with T2DM alone than in the control group(P<0.05).AP2LS,AP3LS,AP4LS,LVGLS,SAXBCS,SAXMCS,SAXACS and LVGCS were significantly lower in patients with T2DM combined with hHcy than in the control group(P<0.05).AP2LS,AP3LS,AP4LS,LVGLS,SAXBCS,SAXMCS,SAXACS,and LVGCS were lower than those in patients with T2DM alone(P<0.05).Conclusion Automated cardiac motion quantification can be a useful addition to the detection of early or insidious cardiac damage in T2DM patients with normal or elevated plasma homocysteine levels.
