Application of Echocardiographic Parameters in the Diagnosis of Elderly Coronary Heart Disease and Evaluation of Coronary Artery Stenosis Degree

Abstract:Objective To explore the application value of echocardiographic parameters in the diagnosis of elderly coronary heart disease and the evaluation of coronary artery stenosis degree.Methods A total of 100 patients with coronary heart disease were selected from the Department of Ultrasound Medicine of the Second People's Hospital of Jiaozuo City and included into the coronary heart disease group,and another 100 healthy volunteers who underwent physical examination in our hospital during the same period were included as the control group.Echocardiography was performed in the two groups,and echocardiographic parameters[left ventricular ejection fraction(LVEF),left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV),and left ventricular end-diastolic diameter(LVEDd)]were compared between the two groups.Coronary angiography was performed on coronary patients,and the angiography results were evaluated according to Gensini scoring criteria.Patients were divided into mild stenosis group(n=45)and moderate and severe stenosis group(n=55)according to the degree of coronary stenosis.Echocardiographic parameters of the two groups were compared.ROC was used to analyze the value of echocardiographic indicators in diagnosing coronary heart disease and evaluating the degree of coronary artery stenosis.Results LVEF in the coronary heart disease group was lower than that in the control group,and LVEDV,LVESV,and LVEDd were higher than those in the control group,with significant differences(P<0.05).Through ROC analysis,the areas under the curve of LVEF,LVEDV,LVESV,and LVEDd in the diagnosis of coronary heart disease were 0.953,0.967,0.986,and 0.976,respectively,and the optimal cut-off values were 51.495,107.500,39.500,and 98.500,respectively.LVEF in the mild stenosis group was higher than that in the control group,and LVEDV,LVESV,and LVEDd were lower than those in the moderate-severe stenosis group,with significant differences(P<0.05).The areas under the curve of LVEF,LVEDV,LVESV,and LVEDd in the evaluation of moderate-severe coronary artery stenosis were 0.937,0.863,0.978,and 0.861,respectively,and the optimal cut-off values were 47.495,123.500,49.500,and 110.500,respectively.Conclusion Echocardiographic parameters are of great significance in the diagnosis of elderly coronary heart disease and the evaluation of coronary artery stenosis degree.
