Relationship Between Oral Hygiene Status,Chronic Diseases and Oral Squamous Cell Carcinoma and Construction of Nomogram Model for Risk Prediction

Abstract:Objective To explore the oral hygiene status and the relationship between chronic diseases and oral squamous cell carcinoma.Methods 160 patients with oral squamous cell carcinoma admitted to the First Affiliated Hospital of Anhui Medical University from May 2014 to August 2023 were selected as the research object,and 200 healthy people who came to the First Affiliated Hospital of Anhui Medical University for physical examination in the same period were selected as the control group.The past disease history,tumor family history,eating habits,oral hygiene,tea drinking history and smoking history of the research object were collected by using the unified questionnaire.Then the unconditional Logistic regression model was used to analyze the correlation between the risk of oral squamous cell carcinoma and chronic diseases and oral hygiene habits,stratified according to gender,drinking status and smoking status,and analyzed the relationship between hypertension,recurrent oral ulcers and poor prostheses and oral squamous cell carcinoma.Results There was no difference in age,sex,nationality,BMI,marital status,family history of tumor,diabetes,hyperlipidemia,diabetes and hyperlipidemia between the two groups(P>0.05).There were differences in education level,occupation,residence,family history of hypertension,bad restorations,current number of teeth,frequency of brushing,dental cycle,recurrent oral ulcer,hypertension,smoking and drinking in the study group(P<0.05).Logistic regression analysis showed that the period of visiting dentists≥5 years/time and the frequency of brushing teeth>1 time/d were the protective factors;recurrent oral ulcer,bad restorations,current number of teeth<20 or 20-27,hypertension,smoking and drinking were risk factors(P<0.05).In Logistic regression analysis,it was found that the protective factors of oral squamous cell carcinoma were≥5 years/visit to the dentist and>1 brushing frequency/d;the risk factors included recurrent oral ulcers,poor restorations and the current number of teeth<20 or 20-27.After stratified analysis,it was found that recurrent oral ulcers could increase the risk of oral squamous cell carcinoma in non-drinkers and non-smokers.At the same time,hypertension could increase the risk of OSCC among non-drinkers and non-smokers;in the female population,the risk of OSCC was higher in patients with hypertension(P<0.05).Conclusion Oral hygiene and chronic diseases are obviously related to the incidence of OSCC.In addition,poor prostheses,recurrent oral ulcers and hypertension may be risk factors for OSCC,and patients in different subgroups may need personalized oral health management strategies.
