Predictive value of Geriatric Nutritional Risk Index and physical activity level for frailty risk in elderly patients with decompensated cirrhosis

Abstract:Objective:To explore the predictive value of Geriatric Nutritional Risk Index and physical activity level on frailty risk in elderly patients with decompensated cirrhosis.Method:Convenience sampling was used to select 152 elderly patients with decompensated cirrhosis from the infectious medicine department of the Sixth Division Hospital of Xinjiang Production and Construction Corps from July 2021 to March 2024.General data questionnaire was used to collect patient information,Liver Frailty Index,Geriatric Nutritional Risk Index and International Physical Activity Questionnaire were used to evaluate patients,and the number of cases of frail patients was counted.The risk factors of frailty in elderly patients with decompensated cirrhosis were screened by univariate and multifactorial binary Logistic regression analysis,and the receiver operating characteristic(ROC)curves of Geriatric Nutritional Risk Index and physical activity level for predicting the risk of frailty in elderly patients with decompensated liver cirrhosis were drawn to evaluate their predictive value.Results:A total of 41 patients had concurrent frailty.Univariate and multivariate Logistic regression analysis showed that age>70 years old,serum albumin,previous fall history,multiple drugs,Geriatric Nutritional Risk Index and physical activity level were risk factors for concurrent frailty in elderly patients with decompensated cirrhosis(P<0.05).The areas under the ROC curves of the geriatric nutritional risk index and physical activity level for predicting the frailty risk of elderly patients with decompensated cirrhosis were 0.850[95%CI(0.774,0.927)]and 0.897[95%CI(0.858,0.936)],respectively.The best truncation values were 88.65 and 619.47 respectively.The maximum Youden indices were 0.602 and 0.619 respectively.It was suggested that both have predictive value.Conclusion:Elderly patients with decompensated cirrhosis have a higher risk of frailty,and the risk factors are complex.Geriatric Nutritional Risk Index and physical activity level have predictive value for frailty risk of elderly patients with decompensated cirrhosis.Clinical evaluation of frailty risk can be carried out according to this,and nutrition and exercise management can be strengthened.
