The influence of fear of disease progression on post-traumatic stress disorder in patients after lung cancer surgery:mediating effect of self-efficacy

Abstract:Objective:To explore the mediating effect of self-efficacy between fear of progression(FoP)and post-traumatic stress disorder(PTSD)in patients after lung cancer surgery.Methods:From January to April 2023,290 primary lung cancer patients who received thoracoscopic surgery in a third grade-A hospital in Tianjin were selected by convenience sampling method and surveyed by using general information questionnaire,Strategies Used by People to Promote Health(SUPPH),Fear of Progression Questionnaire Short Form(FoP-Q-SF)and Post-Traumatic Stress Disorder Checklist-Civilian(PCL-C),and the relationships among self-efficacy,FoP and PTSD were analyzed.Results:The total score of SUPPH was(100.50±22.90),FoP-Q-SF was(29.79±8.34),and PCL-C was(30.68±11.41).PTSD was negatively correlated with self-efficacy(r=-0.364,P<0.001),while positively correlated with FoP(r=0.504,P<0.001),and self-efficacy was negatively correlated with FoP(r=-0.281,P<0.001).The total effect of FoP on PTSD was 0.796[95%CI(0.667,0.925)],the direct effect was 0.724[95%CI(0.549,0.899)],and the indirect effect was 0.072[95%CI(0.011,0.131)].The mediating effect accounted for 9.05%.Conclusions:FoP is a direct predictor of PTSD in patients undergoing lung cancer surgery,and self-efficacy plays a partial mediating role between FoP and PTSD.Medical staff should reduce patients'FoP and enhance their self-efficacy through targeted interventions to improve the post-traumatic stress state.
